Tracking HRT, schedule by schedule
HRT tracking lives or dies on the schedule: the patch-change days, the gel's daily rhythm, the twelve progesterone nights, the phase switch mid-pack. Each guide below carries the sourced cadence facts (SmPCs, BMS, NHS — cadences only, never doses) and what's worth logging around them. The dose conversation belongs to you and your prescriber; the record is yours to build.
Estradiol patch
- Estradiol patch — twice-weekly — Change every 3–4 days — in practice, two fixed weekdays (say, Monday and Thursday).
- Estradiol patch — once-weekly — Change every 7 days, same weekday each week.
Estradiol gel
- Estradiol gel — Once daily, usually at a consistent time.
Estradiol spray
- Estradiol spray — Once daily; the number of sprays is set by your prescriber.
Oral estradiol
- Oral estradiol — Once daily. UK products run continuously; the US Estrace label uses a cyclic pattern (three weeks on, one off) for some indications.
Micronized progesterone
- Micronized progesterone — cyclical — A block of 12 nights (guidelines allow 12–14), at bedtime — anchored either to calendar dates (days 1–12 of each month) or to your cycle (from day 15), per BMS guidance.
- Micronized progesterone — continuous — Every night, at bedtime. This is BMS/NHS guideline practice rather than the product label — worth knowing, not worrying about.
Combined HRT
- Combined patch — continuous — Evorel Conti: change every 3–4 days (two fixed weekdays). FemSeven Conti: change every 7 days. No phase change — every patch is the same.
- Combined patch — sequential — Patch changes twice weekly (Evorel Sequi) or weekly (FemSeven Sequi), with a mid-cycle switch from estrogen-only patches to combined patches.
- Combined oral — continuous — One tablet, once daily, no break.
- Combined oral — sequential — One tablet every day; the composition switches on a product-specific day (Femoston on day 15, Elleste Duet on day 17).
Vaginal (local) estradiol
- Vaginal (local) estradiol — Product-specific: creams and tablets typically start with a short daily phase, then settle into twice-weekly maintenance; rings are replaced every three months. Follow your product's label.
Hormonal IUD (Mirena)
- Hormonal IUD (Mirena) as the progestogen arm — No daily dose. For endometrial protection in HRT: licensed for 4 years (FSRH supports 5 off-label) — different from its 8-year contraception licence.
